Clinic Terms

Please take time to read the following terms carefully. They are important and form the basis of your agreement with The Vitality Clinic. Please print a copy and keep it for your records.

About us
The Vitality Clinic is a trading name of Sharief Ibrahim Limited, a company registered in England with registered number 5479990. Our registered office is at 8 Pool Hill, Bournemouth, Dorset BH2 5PS and our postal address is P O box 600, Walton on Thames, Surrey KT12 9D. What we do and do not do:
We do not provide acute medical care and if you develop acute symptoms (even while you are attending our clinic) you must report back to you GP or local hospital for acute medical treatment.

We complement but do not cancel or replace any other hospital or GP treatment. We also do not replace other conventional, alternative or other medicine such as Chinese medicine. If at any stage alteration of medications becomes necessary then we will (with your permission) write to your GP asking them to execute the changes if they are in full agreement to doing so.

The materials on our web site are provided for information purposes only and our web site does not provide medical or other advice, diagnosis, or treatment. The materials are not intended, and are not be used, as a substitute for professional medical care by a qualified doctor or other suitably qualified medical practitioner. The materials on our web site are not intended, and are not be used, for the diagnosis of any medical condition or the choice of any treatment. The materials on our web site are not comprehensive and may not be based on the most up to date research and medical opinion.

We cannot guarantee any particular outcome with advice and assessment carried on in our clinic because everybody is unique and will assimilate the matter in his or her own styles.

We empower patients to take charge and be the main driver in the healing process and not continue to be passive care recipients as is expected in conventional medical approach. In this context the final outcome depends on the individual commitment, contribution and passion.

Patients enrolled in our clinic cannot assume that their risk of disease will spontaneously vanish, fall back to normal or become reduced. These factors depend on how successful the program is, how bad the baseline condition had been, and how long the condition has been going for.

Patients should understand that our clinic is designed to empower them for health improvement by providing technical, scientific and medical support when they ask for it. In order to achieve success, patients are supposed to be the main drivers of their care and take full responsibility for their choice and overall outcomes.

Arranging appointments

Your appointment request can be submitted to us online or via the telephone. The website uses drop down menus to assist you in entering some of the required information and will also check that you have made entries in all required places. The website does not, however, verify that any information you have entered is correct. You must therefore check the information you have entered carefully for any errors and you will have the opportunity to do this before you submit it. When we have accepted your appointment request and we will issue a confirmation email detailing your appointment. An agreement will exist once we have issued the confirmation email. It is important that you check the confirmation email carefully and if you believe any of the details on it are incorrect you should contact us straightaway.

We keep electronic copies of both your appointment request and our confirmation email for such periods as are required by English law. They are not, however, available for you to view on our website and you should ensure that you print copies for your records.

If any provision of the agreement is declared void, illegal, or unenforceable, the remainder of the contract shall be valid and enforceable to the extent permitted by law.

The English law governs this agreement. If you are resident in the European Union you and we agree to submit to the non-exclusive jurisdiction of the English courts. If you are resident outside the European Union you and we agree to submit to the exclusive jurisdiction of the English courts.

Fees
The fees quoted on our website and in our printed material are in pounds sterling and are subject to change without notice. Prices displayed on our website at any moment in time supersede all other prices previously displayed or that appear in any advertisements, printed material, or other places. Once you have made your appointment, the cost of the appointment will not be subject to any change. This does not apply to invoice errors or omissions. Once a booking has been accepted, offers and discounts cannot be applied retrospectively. Unless otherwise stated, fees include any applicable VAT and other UK taxes.

Payment
All payments must be made in cash, cheque or by credit or debit card. Payment of the deposit is due at the time of booking your appointment and the balance of your account is due on the day of before the start of your appointment. If payment is not made on time we may cancel your appointment and any subsequent appointments and retain your deposit.

Cancellation by you
If you need to cancel your appointment please contact us as soon as you can. We will endeavour to accommodate your new requirements although please note than we are normally unable to accept cancellations for appointments due to take place within 3 working days of the date of the request. If you cancel your appointment less than 3 working days in advance you will be liable for the full fee payable for your appointment unless we are able to make an appointment for another patient at the time we were due to see you (which we unfortunately are only rarely able to do).

Cancellation by us
We reserve the right to cancel your appointment and/or cease to provide services to you if we believe that we are unable to offer services to you, which will be of benefit to you. We will not charge for any appointments cancelled for this reason.

We may also cancel your appointment if we are unable to provide services to you for reasons beyond our control, including unavailability of staff or premises.

Complaints
